Self-pay is accepted. You will be provided with a claim to submit to insurance for possible reimbursement after the consult has concluded.
I do not accept Medicaid or Passport plans.
Please Note: In home visits will include a $25 travel fee charged to the card on file prior to the consult. Self-pay consults will also be charged to the card on file prior to the visit.
Home visits 15 or more miles from 40220 are subject to a $50 travel fee.
For visits over 25 miles from 40220, please contact me directly.
